I was just able to get them. Perhaps they “resupplied” the miles, or maybe GregK neglected to fill in one of the fields properly.

Hm. Odd. I just watched the video again, and was given the form to fill out. Last time it took me right to a page telling me that “due to overwhelming response”, there weren’t any more miles available.
Glad I tried again!

I was not able to get the miles either. After watching the video, got a message saying “Thank you for your interest in the Bose QuietComfort 15 headphones video. Due to the overwhelming response we’ve reached the threshold of mileage awards available for watching the video. You can still earn miles by receiving an in-store demo or with purchase.” Bummer. Don’t bother.
